Indications/Uses
Adjunctive therapy for adults & childn >4 yr who are inadequately controlled on conventional 1st-line antiepileptics for partial onset seizures w/ or w/o secondarily generalized seizures, seizures associated w/ Lennox-Gastaut syndrome, primary generalized tonic clonic seizures. Monotherapy in patients w/ newly diagnosed epilepsy or for conversion to monotherapy in patients w/ epilepsy. Adjunctive therapy for adults & childn (>2 yr) w/ partial-onset or generalized tonic-clonic seizures.
Dosage/Direction for Use
Adult Initially 25-50 mg nightly for 1 wk, increased at wkly intervals by 25-50 mg/day in 2 divided doses. Effective dose: 200-400 mg daily in 2 divided doses. Max: 800 mg daily. Childn ≥4 yr Initially 25 mg for the 1st wk, increased at intervals of 1-2 wk by increments of 1-3 mg/kg/day in 2 divided doses if >25 mg/day. Recommended dose thereafter: 5-9 mg/kg/day in 2 divided doses.
Administration
May be taken with or without food: Swallow whole, do not crush/break/chew.
Contraindications
Hypersensitivity. Pregnancy & lactation. Childn <4 yr.
Special Precautions
Decreased visual acuity &/or ocular pain, myopia, anterior chamber shallowing, ocular hyperemia (redness) & increased IOP; metabolic acidosis. Evaluation of bicarbonate levels during therapy.
Adverse Reactions
Somnolence, nervousness, psychomotor slowing, difficulty w/ memory not otherwise specified, confusion, difficulty w/ conc/attention, anorexia, depression, mood problems, personality changes, insomnia; paresthesia, ataxia, dizziness, speech disorders/related speech & language problems, tremor, hyperkinesias; abnormal vision, diplopia, nystagmus, acute & secondary angle-closure glaucoma; nausea; asthenia, fatigue.
Drug Interactions
May increase plasma conc of phenytoin. Decreased plasma conc w/ phenytoin & carbamazepine. Decreased serum digoxin levels. Increased plasma clearance of OCs. Agents predisposing to nephrolithiasis (renal stone formation).
